How to speed up your care

What to bring or send?  If you don't have copies of your records or the process to obtain your records takes too long, don't worry about it... just book an appointment.  If possible and if readily available, the most helpful documents you could bring or send to us:

  • EKG and labs in the past year

  • List of current medications

  • Reports of radiographic procedures (xrays, CT scans, MRIs) in past few years

  • Records of colonoscopy, endoscopy, cancer related surgeries, cardiac workups

​

If your last practice has an electronic medical record, then check to see if you have access to your records through their portal.  If you can't get the records, just make a request to your clinic and come see us.

​

Another hint, if your labs were done at a major lab company like Quest, LabCorp, or CPL, you can register for a patient account and download your labs from the company websites.  Then send those labs to us electronically... save those trees!

What? No paper registration? No need to download, print, and bring-in completed paperwork...

 

Rapid e-registration is easy as 1-2-3:

​

1. Send us an e-mail, text, phone call with your name and e-mail address.

​

2. Look for our invitation to our patient portal in your e-mail box (check junk mail) and complete the registration process.

​

3. Complete the initial forms (consents, intake, etc.) all electronically!  No NEED to print and bring your forms with you.  We actually prefer you don't print...save some trees.

If you arrive at our office prior to completing the forms, don't worry . . .we'll lend you an Ipad to digitally complete the forms.
